Advocare & Continuum Health Alliance Reach Unprecedented Patient-Centered Medical Home Milestone

78 Practices Achieve Highest Level of Recognition;

70 in NJ, Most of any Medical Group in the State;

100% of PA Advocare Pediatric Practices Receive PCMH Recognition  

Marlton, NJ – March 31, 2015 –Advocare, one of the region’s largest independent medical groups and Continuum Health Alliance (“Continuum”), a physician enablement company, have reached an unprecedented milestone: 78 primary care practices have achieved Level 3 Patient-Centered Medical Home (PCMH) recognition from the National Committee for Quality Assurance (NCQA). Seventy of the Advocare practices are located in New Jersey and represent more PCMH-recognized providers than any other medical group in the state.  All eight Advocare pediatric practices located in Pennsylvania received PCMH recognition.

NCQA accredits and certifies a wide range of health care organizations and manages the evolution of HEDIS®, the performance measurement tool used by more than 90 percent of the nation’s health plans. According to NCQA, the patient-centered medical home is a way of organizing primary care that focuses on care coordination and communication to transform primary care into “what patients want it to be.” Medical homes can lead to higher quality and lower overall costs, and can improve patients’ and providers’ experience of care. Providers who seek PCMH Recognition engage in a rigorous transformation process and must meet six key standards including: provide enhanced access and continuity, identify and manage patient populations, plan and manage patient care, provide self-care support and community resources, track and coordinate care, and measure and improve performance.

“Our physicians, nurses and staff have worked tirelessly over the last two years to embrace the standards set forth by the NCQA. We chose this path because it was the right thing to do for our patients and for our providers, and the results speak for themselves,” said Advocare president, Howard Orel, MD. “Continuum Health Alliance’s Population Health team worked closely with Marc Backal, MD, Chairman of the Strategic Development Committee and Michael Schlitt, MD, Chairman of the Contracting Committee, to provide the coaching and technology platform required to improve quality of care, enhance the patient experience and lower the overall cost of care.”

Advocare providers dramatically altered patient health outcomes and lowered overall cost of care using the Continuum Health Platform, a blend of managed services, best-of-breed technologies and PCMH implementation services. In one shared savings program covering 20,000 patients, 23 Advocare family medicine practices achieved the 90th percentile of care quality, lowered inpatient admissions by 18.8%, reduced 30-day hospital readmissions to 12% (versus a national average of 18%) and lowered the overall cost of care by 17% over 24 months.
