As Senior Vice President of Population Health & Care Innovation for Continuum, Ms. Adams is responsible for driving improved quality, enhanced patient experience and lower overall cost of care among Continuum’s provider customers. She has been instrumental in the successful launch of Consensus Health, a high-performance medical group and MSO for New Jersey’s independent physicians. In less than a year, Consensus Health has exceeded original growth projections and has rapidly expanded throughout the state. A dynamic leader with a passion for transforming healthcare into a consumer-driven system, Ms. Adams balances innovation with execution, enabling Continuum Health’s and Consensus Health’s physician partners to successfully transition from fee-for-service to value-based care.

Prior to joining Continuum, Ms. Adams served as Chief Operating Officer for the Center for Health Innovation and Implementation Science at Indiana University School of Medicine, an organization focused on using agile implementation to support health systems and physician practices better serve their communities through evidence-based care. During this time, Ms. Adams created the nation’s first Graduate Certificate in Innovation and Implementation Science, which trains clinicians and healthcare executives in mastering the skill set of localizing evidence-based practices to solve for daily care challenges. Ms. Adams also served as the Network Director for Great Lakes Practice Transformation Network where she worked with over 2,100 physician practices and 15,000 medical providers serving 10 million patients to advance them through graduated levels of success in value-based care environments.

Ms. Adams was named as one of the Women of Influence honorees for the Indianapolis Business Journal in 2016, as well as one of the top 40 leaders under 40 in Indianapolis, and one of the top 60 Rising National Healthcare Leaders Under 40 by Becker’s Healthcare in 2017. She received a Master of Health Administration from Indiana University Purdue University Indianapolis (IUPUI), and a bachelor’s degree from Indiana University.
